LON External Methods

The Limb Lengthening Over Nail (LON) External Method combines the use of an external fixator with an intramedullary nail for gradual and controlled lengthening. In this technique, an external device such as the Ilizarov fixator or a monorail system is temporarily attached to guide and stabilize the bone during the distraction phase. Once the desired length is achieved, the external fixator is removed, leaving the internal nail in place to support the healing bone.

LON Tibia (Ilizarov Method):

Lengthening Limit: 5 cm to 7 cm

Advantages:

    • Low cost
    • Safest, most trusted, and reliable method
    • Minimal chance of infection
    • Moderate, manageable pain with medications
    • Smaller scars
    • Comfortable sleeping positions: straight and sideways
    • Partial weight-bearing during the lengthening phase

Recovery Timeline:

    • Normal walking: 6-7 months
    • Jogging: 9-10 months
    • Running: 12 months
    • Athletic ability: 2 years

    • Requires two surgeries, leading to higher costs
    • Higher risk of infection in the femur, lower risk in the tibia
    • Higher pain levels in the femur, moderate in the tibia
    • Longer scars on the femur and smaller scars on the tibia
    • Less comfortable sleeping (straight or opposite side of operated femur)
    • Partial weight-bearing during the lengthening phase

    • Normal walking: 10-12 months

LON Internal Methods

The Limb Lengthening Over Nail (LON) Internal Method is a cutting-edge procedure that eliminates the need for external fixators by utilizing advanced internal devices like the Precice 2.2 nail or other motorized internal systems. During the surgery, the bone is carefully cut, and a telescopic nail is inserted inside. This nail is controlled externally via magnetic or mechanical adjustments, enabling precise and discreet limb lengthening.

Risks and Complications of Limb Lengthening Surgery

Limb lengthening is a complex surgical procedure, and like any surgery, it carries potential risks and complications. While most complications are rare, it’s important to be aware of potential issues that may arise during or after the procedure. Additional complications not listed here may occur on rare occasions.

Description: New bone formation may take longer than expected, delaying the healing process.
Prevention: Maintaining a healthy diet and taking prescribed supplements can help promote bone growth.
Treatment: In most cases, this issue can be resolved by following dietary recommendations and taking supplements as advised.

Description: The new bone tissue grows too quickly, closing the gap before the desired length is achieved.
Prevention: Regular monitoring of the bone-lengthening process to adjust the distraction rate as necessary. Increasing the rate of lengthening (up to 1 mm/day) temporarily can prevent early joining.
Treatment: If consolidation has already occurred, surgical intervention may be required to re-break the bone.

Description: The broken bone fails to heal or connect properly, often caused by insufficient blood supply, infection, premature weight-bearing, or surgical errors.
Prevention: Selecting an experienced surgeon, adhering to dietary plans, using supplements, and utilizing ultrasonic stimulation devices to encourage bone healing.
Treatment: Surgical intervention such as bone grafting may be required.

Description: The growth of germs or bacteria at the surgical or pin sites.
Prevention: Maintain good hygiene and clean the surgical site regularly.
Treatment: Managed with medications or, in severe cases, surgical intervention.

Description: The foot remains pointed downward, making it difficult to pull the foot and ankle upwards. Common after lengthening the tibia by more than 5 cm.
Prevention: Regular and rigorous physiotherapy during the process.
Treatment: A minor surgical procedure called percutaneous Achilles tenotomy may be required.

Description: Nerve damage caused by overstretching during fast lengthening or due to pin placement.
Prevention: Keep the lengthening rate below 1 mm per day, especially after achieving 6 cm in length.
Treatment: Initial treatment involves medications. If required, minor nerve decompression surgery can be performed for fast recovery.

Description: A rare complication where fat or bone marrow enters the bloodstream during surgery, potentially affecting the lungs.
Prevention:

  • Careful surgical planning to minimize bone marrow and fat release.
  • Use of specialized tools to suction bone fragments and reduce bone dust.
  • Prescription of Xarelto 10mg to manage fat embolism risk.
    Note: To date, none of our patients have experienced fat embolism.
